Position description
This position will be approximately 50% Palliative Care Inpatient & 50% Hospice.

The Nurse Practitioner is part of a strong interdisciplinary Hospice and Palliative Care team which includes 3 physicians, 6 nurse practitioners, registered nurses, social workers, chaplains and others who enjoy working together toward a common goal. The NP cares for patients and their families who are experiencing advanced, life-threatening illness and helps them live as well as possible through excellent symptom management, advance care planning and psychosocial support. The Palliative & Supportive Care Service has been identified as a key area for growth organizationally and regionally.

Palliative Care practice encompasses physical assessment, diagnosis, treatment and evaluation with special emphasis on excellent symptom management and disease-specific counseling, as well as assessment and coordination of social, emotional, spiritual, and practical needs. The focus of the Palliative Care NP is not on treating a particular illness but treating the whole person and family who are experiencing it through the course of the illness to the end of life.

With the hospice population, the NP works in collaboration with an interdisciplinary health care team to provide exceptional treatment and support in the final stages of life. The NP will provide holistic care in the familiar setting of the patients home, whether it is a private residence, group home, assisted living, or a skilled nursing home. The NP will be responsible for in-person hospice recertification visits, symptom management, goals of care discussion, rounding on inpatient hospice patients, as well as being available for clinical consultation with the hospice team.

Active Wisconsin R.N. license or eligibility to practice professional nursing in the State of Wisconsin. National certification (i.e. ANCC or AANP) as a Nurse Practitioner. Wisconsin state APNP certification.

Certification in Hospice and Palliative Care (CPHN or ACPHN) preferred but not required at time of hire.
