No productivity expectations! We welcome new graduates to apply! Position Type: Full-Time Salary Range: $45 - $55 per hour based on experience. Bonus: $5,000 Location: West Region: Chaska, Waconia, and Maple Plain Responsibilities of the Physical Therapist: Assessment & Diagnosis: Evaluate patients' physical conditions, mobility, and medical histories to create tailored treatment plans. Plan Development & Implementation: Design and execute rehabilitation programs aimed at enhancing patients' functional abilities. Hands-on Therapy: Deliver manual therapy, therapeutic exercises, and utilize modalities such as ultrasound and electrical stimulation as necessary. Patient Education & Support: Educate patients, caregivers, and staff on clinically relevant topics, including pain management strategies and fall prevention. Instruct patients and families on exercises and injury prevention techniques. Progress Monitoring & Plan Adjustment: Track patient outcomes, document advancements, and modify treatment plans as required. Collaboration with Healthcare Teams: Partner with physicians, nurses, and other therapists to ensure holistic patient care. Qualifications for the Physical Therapist: Graduation from an accredited Physical Therapy program and possession of a bachelor’s degree. Current Minnesota Physical Therapist License from an accredited institution. Strong understanding of state, federal, and local regulations related to home health care. Ability to perform essential job functions, with or without reasonable accommodations.
