Shift Type: 2:00 PM - 10:00 PM, Full Time PM shift, includes every
other weekend (specific weekdays vary by facility schedule) Hours

promptly and clearly with physicians, families, and the care team
about resident condition changes Complete resident assessments
thoroughly and on time to ensure personalized care plans Administer
medications accurately following the 5 Rights and facility
protocols Perform wound care and monitor healing, adjusting care as
needed Assist CNAs and lead the floor when clinical direction is
required Monitor residents for subtle changes and intervene or
escalate appropriately Maintain accurate, timely charting and
documentation for resident safety and compliance Report any
allegations of abuse, neglect, or misappropriation to leadership
immediately Support other duties as requested to ensure
high-quality resident care Qualifications Current Wisconsin
Registered Nurse (RN) license required Associate degree in nursing
(two-year degree) required; BSN preferred Current CPR/BLS
certification required 1 year(s) of related long-term care or
nursing experience preferred Strong attention to detail and
accuracy in clinical tasks and documentation Excellent
communication and care coordination skills; able to lead on the
floor Education Associate Degree in Nursing (required) Bachelor of
Science in Nursing (preferred) Knowledge & Skills Medication
administration and safe practices Wound care assessment and
treatment Resident assessment and care planning Thorough charting
and documentation skills Care coordination and clear communication
with interdisciplinary teams Ability to supervise and mentor CNAs
and other staff Knowledge of long-term care regulations and
