Requirements: Clinical Interview required Minimum two years recent
experience required in position applying for. New Grads & Residents
not accepted. ABEM/AOBEM Board Certificate or Board eligibility
letter Clean file/background with no malpractice Procedure logs
(last 2 years) Updated CV, driver???s license, color photo, state
medical license (including licensure held in other states), CSR,
DEA, education certificates, 5 year malpractice history, need
narrative & legal documentation if provider has claims, ECFMG if
applicable, ABEM/AOBEM Board Certificate, Board eligibility letter,
TB/PPD/Quantiferon/Chest x-ray (within 12 months), COVID, and FLU
