Job Description:

Aveanna Healthcare, a prominent national provider of tailored home healthcare, invites compassionate and skillful registered nurses to apply for the position of Home Health Registered Nurse. In this vital role, you will be integral in providing the highest quality of nursing care to a diverse patient population directly in their homes. Collaborate with a supportive and dedicated team to facilitate optimal patient outcomes through tailored care plans. This position combines expertise in nursing with a passion for improving patient quality of life in the comforting environment of their own homes. Conduct patient assessments and evaluate patient needs collaboratively with healthcare teams. Develop and implement individualized care plans based on patient's health status and goals. Administer medications, wound care, and other nursing services according to physician’s orders and care plans. Educate patients and their families on disease processes, medication management, nutrition, and home care techniques. Monitor patient health and coordinate with healthcare teams for updates or changes in condition. Ensure compliance with all health and safety regulations within a home health setting. Strong interpersonal and communication skills to interact effectively with patients and families. Ability to work autonomously in a home-setting environment. Proficiency in electronic health records (EHR) systems. Adept in time management to efficiently schedule and prioritize patient visits. Knowledge of proper safety protocols and medical procedures. Medical Insurance Dental Insurance Vision Insurance 401k Current Registered Nurse (RN) license in the operating state. CPR/BLS certification. Valid driver’s license and reliable transportation as this role requires home visits.
