Description
Lutheran Social Services is currently seeking a Care Coordinator
with a SAC IT for its Renewed Strength Program. This is part time
position, 30 hours a week. Due to contractual requirements, only
female applicants will be considered at this time. This role will
primarily be working in the Eau Claire area.
Only Individuals with a Substance Abuse Counselor/In Training
license will be considered for this role. This is a Benefit
Eligible Role!
The purpose of this program is to provide comprehensive,
trauma-responsive, culturally and linguistically relevant substance
use disorder (SUD) treatment services for women, in urban and rural
settings. This program will aim to reduce the prevalence of alcohol
and other substance use disorders and increase behavioral health
equity among pregnant and postpartum women, and women with
dependent children. The program is grant funded and its primary
focus is to provide comprehensive SUD treatment, intervention, and
recovery support services to pregnant, postpartum, and parenting
women, with pregnant women being a priority as indicated in 45 CFR
96.120-137, Wis. Stat. * 51.46 and 51.42(3).
Under direction, performs work involving the assessment, service
planning, and implementation of the service plan for assigned
individuals. Performs other duties as required.

CERTIFICATES, LICENSES, REGISTRATIONS:
The incumbent of this position must have a valid driver's license
and have reliable transportation to perform the essential duties of
the role; a motor vehicle check (MVR) with a satisfactory driving
record per the LSS Driver Safety Procedure is required, and ability
to meet LSS auto insurance requirements.
